This profession is regulated: access is decided not by an employer but by a competent authority — the one in your federal state. The yardstick is § 9 BQFG: your qualification counts as equivalent if there are no substantial differences from the German training.
Nursing is the profession where partial recognition is the norm rather than the exception — and that is not bad news. The system is built for follow-up qualification: the decision names your gaps, and for exactly those gaps there are courses, funding and a dedicated residence permit.

What to do
- Apply to the competent authority of your federal state under § 2 PflBG — the real goal is permission to use the professional title.
- Read the decision as a map: it lists the missing modules, not the whole training. Only those need closing.
- Choose the path: an adaptation course (practical, on the job) or the Kenntnisprüfung. Where you have the choice, the course is the safer route.
- Still abroad? § 16d (3) AufenthG opens the recognition partnership: enter, work in a qualified job and run the procedure here — conditional on a written agreement with the employer.
Where people get stuck
The bottleneck is almost never the professional content but the language and the contract. Employers in care recruit actively abroad — read the § 16d (3) agreement closely: it obliges the employer to let you complete the qualification within the employment relationship. That is your right, not their favour.
If you still need to immigrate
Important if you still need to immigrate: for the skilled-worker visa under § 18a AufenthG the qualification alone is not enough — § 18 (2) no. 4 AufenthG requires that equivalence HAS been established. You may not need recognition to work, but you do need it for the visa.

Partial recognition — have I lost?
No. In nursing this is the standard branch. The decision is a list of gaps together with the route: course or examination. The Anerkennungszuschuss pays up to €3,000 for the qualification if your income is below €32,000 gross a year (€50,000 for married couples).
May I work in care while the procedure runs?
Usually yes, but as a care assistant and not under the protected title. Calling yourself Pflegefachfrau or Pflegefachmann requires the permission under § 2 PflBG.
How long does the procedure take?
The authority must decide within three months of complete documents (§ 6 (3) BQFG) and confirm receipt within one month (§ 6 (2)). It may extend once — with reasons, communicated in good time. The follow-up qualification afterwards usually takes months, not weeks.
