Who can use this route?

The law provides it for late resettlers and persons treated as equivalent. It is not a general procedure for everyone who moved here: the group is defined by statute, and the registry office checks membership against admission documents.

What exactly can be changed?

The law permits several steps: dropping a patronymic, changing a gender-specific ending of the family name, adopting the German form of a first name and, where none exists, choosing a new one. It is a catalogue of permitted changes, not a free choice of any name.

How does this differ from an ordinary name change?

No good cause has to be proven. The public-law procedure requires an important reason and a decision by the authority on the merits. Here a declaration within the statutory framework is enough.

Does it affect the children too?

It can, and that deserves thought in advance. A change of family name often extends to minor children, and older children may need to consent. The registry office will explain the exact mechanics for your case.
